Expat health insurance in Indonesia — BPJS, local or international

Three options for expats living in Bali: BPJS Kesehatan, a plan from an Indonesian insurer, or an international plan. What the regulation and the insurers' own documents say about who can join, cover area, cashless hospitals, evacuation and renewal, checked Oct 2026.

The short version

Three ways to be insured in Bali. They fail in different places.

An expat living in Bali has three options: BPJS Kesehatan, the national scheme; a plan from an Indonesian insurer; or an international plan issued offshore. This page sets them side by side on the points that decide a bad week: who can join, where the cover applies, whether the hospital bills the insurer, what the policy says about evacuation, and what happens at renewal.

We read the regulation text and the insurers' own documents in October 2026. We picked two local plans (Allianz Flexi Medical and AXA Health Protector) and two international ones (Cigna Global and SafetyWing Complete) as worked examples. That is not a ranking and not the whole market, and where a document did not say something we say so rather than fill the gap.

Our reading, not a source
For someone living in Bali, the useful question is usually not "BPJS or private" but whether BPJS applies to you at all, and which private plan sits next to it. The regulation text we read names foreigners who work in Indonesia for six months or more as BPJS participants. The decision flow further down is ours, built on the facts in the table, and none of the documents recommends it.
What we checked

Sources and dates

  • BPJS: Perpres 59/2024 (third amendment to Perpres 82/2018 on Jaminan Kesehatan) and Perpres 64/2020, both from the BPK legal database, plus Law 24/2011 Article 14.
  • Allianz: the Allianz Flexi Medical product summary (RIPLAY) on allianz.co.id, edition dated 12 June 2024, Allianz's Medical Assistance page, and its hospital-list and Blue Eagle Network pages.
  • AXA: the AXA Health Protector product summary (RIPLAY, file created August 2026) and AXA's hospital list for its individual products, updated 8 October 2026.
  • Cigna: Cigna's Indonesia page, its Global Health Options customer guide (PDF marked 02/25) and the underwriter statement on cignaglobal.com.
  • SafetyWing: the Nomad Insurance plan page.

Prices below are what those documents show, with the month we checked (October 2026). They are not quotes, they are not like for like, and we have not converted rupiah into dollars or back.

Option 1

BPJS Kesehatan: what the regulation says

The definition of a participant in the current regulation (Perpres 59/2024, Article 1 point 2, restating the 2018 text) reads: "Peserta adalah setiap orang, termasuk orang asing yang bekerja paling singkat 6 (enam) bulan di Indonesia, yang telah membayar Iuran Jaminan Kesehatan." Our translation: a participant is any person, including a foreigner who works in Indonesia for at least six months, who has paid the health insurance contribution. Law 24/2011 Article 14 (the BPK text we read is the original enactment; we did not check later amendments) uses the same six-month wording for the duty to join the social security programme.

Both texts say works. Neither says "holds a KITAS". We did not find a rule in these texts on whether a foreigner who does not work, such as a retiree or a spouse, can join, so ask BPJS Kesehatan about your own permit before you plan around it.

  • Contribution for wage earners: 5% of monthly wages, 4% paid by the employer and 1% by the employee (Perpres 64/2020, Article 30), calculated on wages up to Rp12,000,000 a month (Perpres 59/2024, Article 32). Private-sector employees are in this category (Perpres 82/2018, Article 4(2)(g)). We did not look at the self-paid categories.
  • Where it applies: Article 52 lists services that are not covered. They include treatment performed abroad, and treatment at facilities that have no agreement with BPJS Kesehatan, except in an emergency.
  • Ambulance: the regulation defines the ambulance benefit (by land or water) as transport of a referred patient, in certain conditions, between health facilities.
  • Pre-existing conditions: the Article 52 list of services that are not covered does not include them. Private plans are different, as the local plan summaries below show; our guide on pre-existing conditions covers how to handle that.
Option 2

Plans from Indonesian insurers: two we read

Allianz Flexi Medical

Issued by PT Asuransi Allianz Life Indonesia, which states in the summary that it is licensed and supervised by the OJK. It is an individual health policy and the summary lists agency as its sales channel.

  • Who: policyholder from 18, insured person from 1 month to 75 years. The application asks for a KTP, KITAS or KIMS; the claim documents list a passport for foreign adults.
  • Cover area: six plans. Classic and Classic Plus cover Indonesia; Essential Pro and Essential Plus cover Asia except Hong Kong, Singapore and Japan; Elite Pro and Elite Plus cover Asia plus Australia. Emergency or accident care outside the plan area is listed as worldwide except the USA, within 48 hours of the event.
  • Limits: annual benefit limit of Rp10 billion on the two Classic plans, Rp15 billion on the two Essential plans and Rp20 billion on the two Elite plans.
  • Price: the summary's worked example is a 30-year-old on Essential Pro with the deductible option: Rp9,124,000 a year. Premium rises with the insured person's age at renewal. Checked October 2026; an illustration, not a quote.
  • What it leaves out: pre-existing conditions are excluded. Outpatient care is on the exclusion list apart from care after an accident or in an emergency, although the benefit table separately lists fees in the 60 days before and 90 days after a hospital stay. Dental care is excluded except after an accident, and pregnancy is excluded. A 30-day waiting period applies to most benefits, and a list of named illnesses that includes diabetes, hypertension and heart disease is not covered until the policy has run for 12 consecutive months.
  • Cashless: the summary describes a cashless facility for inpatient care at Allianz's hospital network. Allianz's hospital-list page does not mention Flexi Medical. It links to a Blue Eagle Network page that names three Bali hospitals (RS Surya Husadha Denpasar, RS Surya Husadha Ubung and RS Surya Husadha Nusa Dua), says Allianz customers can still be treated cashless at all Allianz partner hospitals, and lists the same three extras for each of the three Bali hospitals: a free one-class room upgrade, an exclusive waiting room for check-up and non-check-up patients, and a 5% check-up discount (some are marked with an asterisk on the page, and the page dates the inpatient extras from 2 September 2024). Separately, its general benefits section for the network mentions priority at registration and discharge. That page does not name Flexi Medical either, so ask Allianz whether it applies to your plan.
  • Evacuation: the summary includes a Medical Assistance service that applies when you are travelling more than 100 km from your residence, or are abroad for a period not exceeding 90 days. Allianz's service page lists cases where it will not evacuate, including a move from one medical facility to another.
  • Alongside BPJS: the deductible is worked out after benefits already paid by another insurer or by BPJS, and care already reimbursed by the government or another health insurer is excluded.

AXA Health Protector

Issued by PT AXA Financial Indonesia. The summary says the product has OJK approval, and AXA's product page lists agency as the distribution channel.

  • Who: insured person from 0 to 80 years, policyholder from 17 to 80. The insured person can be Indonesian or a foreigner who meets the requirements to live in Indonesia; foreigners give a passport and a KITAS or KITAP.
  • Cover area: Classic Lite, Classic and Classic Plus cover Indonesia; Premium and Premium Plus cover Asia except Singapore, Hong Kong and Japan; Business Plus covers Asia; Infinite Plus (with the XtraSave feature) covers the world except the USA. Emergency inpatient care outside the plan area is listed for up to 90 days. For other care outside the plan area the summary pays a percentage of eligible costs: Classic Lite, Classic and Classic Plus pay 60% in Asia (excluding Singapore, Hong Kong and Japan), 20% in Singapore, Hong Kong and Japan, and 0% in the rest of the world excluding the USA and Asia; Premium and Premium Plus pay 100%, 30% and 0%; Business Plus pays 100%, 100% and 60%; Infinite Plus pays 100% in all three.
  • Limits and price: annual benefit limits from Rp10 billion to Rp30 billion by plan. The summary gives the premium as starting from Rp6,419,900 a year (checked October 2026; the summary does not say for which plan or age).
  • Renewal: one-year period that can be extended up to age 99.
  • Outpatient: the summary's outpatient list is: care before and after a hospital stay; physiotherapy, occupational and speech therapy; cancer treatment and cancer monitoring; dialysis; palliative care; traditional Chinese medicine; psychology consultations; outpatient care after an accident (including dental and follow-up visits); daily care; and surgery received as an outpatient. Under "other benefits" it separately lists outpatient costs for chronic illness that is not a pre-existing condition, shown as not included on Classic Lite, Classic and Classic Plus. It lists no routine GP visit benefit.
  • What it leaves out: pre-existing conditions are the first exclusion. Hospitalisation or surgery for a named list of illnesses (including diabetes, high blood pressure and heart disease) is excluded in the first 12 months, and there is a 30-day waiting period.
  • Cashless: AXA's hospital list for its individual products (updated 8 October 2026) has a cashless column. It has 47 Bali entries and marks 22 of them "Ya", including BIMC Hospital Kuta and Siloam Hospitals Bali. The list says it applies to products abbreviated MOC, MEC, AHP, FFH and EH, and to the AHP rider; it does not spell the abbreviations out, and the AXA Health Protector summary itself never uses "AHP". Reading AHP as AXA Health Protector is our inference, so confirm with AXA that the list applies to your plan. For plans with XtraSave, the list also marks hospitals preferred or non-preferred; the list says that care at a non-preferred hospital is claimed by reimbursement with co-payment.
  • Evacuation: the summary lists an emergency medical evacuation service, provided by a provider AXA appoints. It gives no limits or conditions for it.
Check the plan name
Our own comparison table and provider list are editorial profiles, not quotes, and they can lag the insurers' documents. Take the plan name, the cover area line and the hospital list from the insurer's current documents before you buy, and get the hospital list in writing for the plan you choose.
Option 3

International plans: issued offshore

Cigna Global

Cigna's site says its policies are underwritten by Cigna Global Insurance Company Limited, a Guernsey company authorised and regulated by the Guernsey Financial Services Commission. Its Indonesia page quotes Silver, Gold and Platinum plans, and also lists a regional plan called Close Care that we did not read.

  • Plans and limits: Silver with an annual benefit limit of US$1,000,000, Gold with US$2,000,000 and Platinum shown as "paid in full". Each comes with worldwide cover or worldwide cover excluding the USA.
  • Add-on modules: outpatient, Evacuation and Crisis Assistance Plus, health and wellbeing, and vision and dental.
  • Evacuation: the customer guide says cover for medical evacuation or repatriation is only available if you have the Evacuation and Crisis Assistance Plus option.
  • Price: the Indonesia page does not publish premiums; you get a quote.
  • Cashless: the customer guide says that for most inpatient and day-patient treatment Cigna pays the provider directly, through a direct billing agreement or a guarantee of payment issued after prior authorisation, and that in some countries it cannot pay a provider directly. It points members to a provider search tool. We did not find a Bali hospital list on the pages we read.
  • Renewal: the period of cover is 3 to 12 months and renewable. Cigna sends a renewal invite 45 days before the renewal date, says you are not re-underwritten at renewal if you make no material change, and says it does not base the renewal premium on claims you did or did not make.

SafetyWing Complete

SafetyWing's Nomad Insurance comes as Essential and Complete. The plan page describes Complete as full health insurance with extra travel protections, with ongoing coverage that is "renewable indefinitely" and routine and emergency medical care anywhere in the world. It lists the price as US$177.50 a month for ages 18 to 39 (checked October 2026; other ages have other prices in its calculator). The page says you can sign up at any point during your travels or while living abroad, and that cover spans 180+ countries with US coverage extra. The footer says the plans are underwritten by SafetyWing Insurance I.I., a Puerto Rico domiciled international insurer.

Two limits of what we can say. The page describes Complete as everything in Essential plus extras, and the Essential evacuation line is US$100,000 lifetime maximum, so check Complete's own evacuation limit in the policy before relying on it. And the Complete plan page does not say whether hospitals bill SafetyWing directly. Our three-plan comparison says SafetyWing calls Essential a reimbursement-based plan and that for inpatient stays it can sometimes arrange a guarantee of payment, case by case; that is about Essential, not Complete. Claims rules, pre-existing conditions and scooter terms for these plans are in our three-plan comparison, which also covers Genki and Insured Nomads.

These cover different things in different currencies, so we do not rank them by price.

Decision flow

Which one first? Our reading, step by step

This is our own sorting logic, built on the table above. It is not a rule, and a broker or the insurer may see your case differently.

  1. Working in Indonesia for six months or more? → Start with BPJS. The regulation text names you; ask BPJS Kesehatan how it applies to your permit and register. If you do not work, the texts we read do not settle it, so ask BPJS before you assume either way.
  2. Want a Bali private hospital to bill the insurer? → Start with AXA Health Protector, because it is the plan where we found a hospital list marking Bali hospitals cashless. Caveat: that list is coded "AHP" and the summary never uses the code, so tying it to this plan is our inference. Allianz's Blue Eagle page names three Bali hospitals but not Flexi Medical, so get the list for your exact plan in writing before you pay. Same advice for Cigna.
  3. Want cover outside Asia, or evacuation written into the policy? → Start with Cigna Global plus its evacuation module (its guide says evacuation cover exists only with that option), or SafetyWing Complete, which is worldwide on its page; check its evacuation limit first. Among the local plans, AXA's Infinite Plus covers the world except the USA, while Allianz's widest area is Asia plus Australia (with emergency care elsewhere within 48 hours).
  4. Got a condition you already know about? → If you work in Indonesia for six months or more, start with BPJS, whose not-covered list does not mention pre-existing conditions. If you do not, no plan we read lets you skip the question: Allianz and AXA both list pre-existing conditions as an exclusion, and we did not read pre-existing terms for Cigna or SafetyWing Complete. Ask the insurers to underwrite your condition in writing, and read the pre-existing conditions guide before you apply to anyone. Disclose it.
  5. Not working in Indonesia for six months or more? → BPJS may not be your starting point. Our nomad visa insurance guide covers the short-stay options.

How BPJS and a private plan fit together, including what each pays first, is in our BPJS vs private guide, and how cashless claims work is in the cashless vs reimbursement guide. To see current plan details side by side, use the comparison tool, the provider profiles, or ask for a health quote.

Before you pay

Four things to get in writing

  • The hospital list for your exact plan name, with the Bali hospitals you would use marked cashless or not. A product summary can describe a cashless facility without listing a single hospital.
  • The evacuation conditions. Allianz's service applies when you are travelling more than 100 km from your residence or abroad; Cigna sells it as a module; AXA lists the service without conditions in the summary. Ask what applies if you are treated in Bali and need to go to another hospital or country.
  • What your KITAS type and age do to eligibility, and which documents the insurer wants.
  • What happens at renewal: whether premium moves with age, and whether the insurer re-underwrites you.
FAQ

Common questions

Can a foreigner join BPJS Kesehatan?
The regulation text defines a participant as any person, including a foreigner who works in Indonesia for at least six months, who has paid the contribution. We did not find text on foreigners who do not work, so ask BPJS Kesehatan about your own permit.
Does BPJS cover me outside Indonesia or fly me out?
Its list of services that are not covered includes treatment performed abroad, and the ambulance benefit is defined as transport of a referred patient, in certain conditions, between health facilities, by land or water. For evacuation you would look at a private or international policy.
Can I buy an Indonesian health plan on a KITAS?
The two summaries we read both mention foreign applicants: Allianz asks for a KTP, KITAS or KIMS, and AXA asks foreigners for a passport and a KITAS or KITAP. We did not read either insurer’s underwriting rules, so confirm your permit type and age with the insurer.
Who stands behind an international plan?
On their own sites, Cigna Global policies are underwritten by a Guernsey company regulated by the Guernsey Financial Services Commission, and SafetyWing plans by a Puerto Rico domiciled international insurer. Ask which regulator supervises the insurer named on your policy before you buy.
If I have BPJS and a private plan, who pays?
It depends on the policy. Allianz’s summary works out its deductible after benefits paid by another insurer or by BPJS, and excludes care already reimbursed by the government or another health insurer. AXA’s summary asks for the first insurer’s payment statement when benefits are coordinated. Ask your insurer how it treats a BPJS claim before you rely on both.
What does it cost?
The documents give one example each, in different currencies and for different plans: Allianz Rp9,124,000 a year for a 30-year-old on Essential Pro with the deductible option, AXA from Rp6,419,900 a year, SafetyWing Complete US$177.50 a month at ages 18 to 39, and Cigna by quote. For employees BPJS is 5% of wages up to Rp12,000,000 a month, split 4% employer and 1% employee. Checked October 2026.
